Enrollment Information SOP
Purpose
To ensure all required enrollment information is properly collected, documented, maintained, and available for review in compliance with Florida child care licensing regulations.
Scope
This SOP applies to all facility operators, directors, and administrative personnel responsible for enrolling children and maintaining enrollment records.
Regulatory Reference
Enrollment information requirements are established by the Florida Administrative Code and enforced by theĀ Florida Department of Children and Families.
Procedure
Step 1: Collection of Enrollment Information

Enrollment information must be obtained from the childās custodial parent or legal guardianĀ prior to accepting a child into care.

Information must be documented onĀ CF-FSP Form 5219, Child Care Application for Enrollment, or an equivalent form containing all required information as outlined by the Department.

CF-FSP Form 5219 may be obtained from the licensing authority or the Departmentās child care website.
Step 2: Maintenance of Enrollment Records

All enrollment records must be:

Kept on file
-
Maintained as current
-
Available for review by the licensing authority at all times

Step 3: Authorization for Child Release

Enrollment documentation must includeĀ written permissionĀ authorizing the facility to release the child only to individuals approved by the custodial parent or legal guardian.

The following information must be documented for all authorized individuals:

Full name

Address

Phone number

Step 4: Required Parent/Guardian Acknowledgements
The facility must obtainĀ signed statementsĀ from the custodial parent or legal guardian confirming receipt of the following:

Know Your Child Care Facility Brochure

CF/PI 175-24, or an approved equivalent provided by a local licensing agency

Disciplinary and Expulsion Policies

The facilityās written disciplinary and expulsion policies

Food and Nutrition Policies
-
Including food safety procedures and food allergen information

Influenza Virus Information
-
ProvidedĀ annually during August and September

Step 5: Consent for Records Access

Enrollment information must includeĀ written parental or guardian consentĀ authorizing child care personnel to access the childās records as required for care, supervision, and regulatory compliance.
Compliance
Failure to collect, maintain, or properly document enrollment information may result in licensing violations. All enrollment records must be complete and accessible at all times for inspection.
