Purpose:
To ensure all diaper changes are documented accurately and consistently, and to provide a professional, transparent response when a diaper change was completed but not recorded.

SITUATION

A diaper change was completed, but:

  • It was not documented in the app or paper log

  • The parent notices the missing entry

  • Trust and confidence in daily care is impacted

CORE PRINCIPLES

  • Documentation is part of care—not optional

  • Parents rely on records for health and routine

  • Missing documentation must be addressed immediately

  • Staff respond with ownership, not excuses

STEP-BY-STEP PROCEDURE

STEP 1: When the Missed Documentation Is Discovered

Teacher or Staff Responsibilities

  • Notify the Director immediately

  • Confirm:

    • Approximate time of diaper change

    • Staff member who completed it

    • Condition of the child at the time

Important:
Never backdate or guess exact times if unsure.

STEP 2: Correct the Record (If Allowed by Policy)

Director Responsibilities

  • Determine if the system allows a late entry with a note

  • Add clarification such as:

Approved Documentation Language:
“Diaper change was completed but was not documented at the time. Entry added to ensure accurate daily care records.”

STEP 3: Parent Communication

Who Communicates: Director or Lead Teacher

Approved Language (App or Verbal):
“I wanted to let you know that a diaper change was completed today, but it was not entered in the app at the time. We’ve addressed the documentation gap and want to thank you for your understanding.”

Do NOT Say:

  • “We forgot.”

  • “It was busy.”

  • “The app wasn’t working.”

STEP 4: Staff Coaching (Private)

Director Responsibilities

  • Address the missed documentation with staff

  • Reinforce:

    • Diaper change = immediate documentation

    • Licensing and parent trust implications

  • Document coaching if needed

DAILY PREVENTION EXPECTATIONS

  • Diaper changes must be logged immediately after completion

  • Substitutes receive documentation training at shift start

  • End-of-day check: diaper log reviewed before dismissal

WHAT STAFF MUST NEVER DO

  • Guess times

  • Backdate without notation

  • Minimize the issue

  • Blame the parent for noticing

  • Ignore missing entries
