To ensure that each enrolled child has a complete and current file that meets state licensing requirements and supports health, safety, and emergency preparedness at the center.

This SOP applies to administrative staff, enrollment coordinators, and center directors responsible for collecting, organizing, and maintaining all child enrollment and health documentation.


 

šŸ› ļø Procedures

1. Initial File Creation at Enrollment

  • Create a physical or digital folder for each child upon registration.

  • Collect and verify the following required documents before the child’s first day:

    • Completed enrollment form with emergency contacts

    • Current immunization record or exemption form

    • Health form or physical signed by a physician (as required by state law)

    • Allergy and medical action plans (if applicable)

    • Authorized pick-up list

    • Signed parent handbook acknowledgment

    • Transportation permission, media release, and field trip consent forms

    • Food program paperwork (if participating in CACFP or similar)


 

2. Ongoing File Maintenance

  • Review each child’s file monthly for accuracy and completeness.

  • Set reminders for key expiration or renewal items:

    • Annual health form updates

    • Updated emergency contact or pick-up information

    • Expiring medical plans or allergy documentation

  • Notify parents 30 days in advance of any missing or expiring documents.

  • Follow up weekly until updated forms are received.


 

3. Storage and Security

  • Store files in a secure, locked cabinet or approved digital management system.

  • Separate confidential health information into a designated health file.

  • Ensure only authorized personnel have access to child records.


 

4. Licensing Compliance and Readiness

  • Maintain all child files in a format that is easy to audit during inspections.

  • Use a Child File Compliance Checklist for each file.

  • Prepare a ā€œLicensing Binderā€ or digital folder with all active enrollment files for easy access during visits.


 

5. Withdrawal or Transition Procedures

  • When a child withdraws:

    • File the record in the ā€œFormer Studentsā€ archive

    • Retain records for the period required by state licensing (e.g., 3–5 years)

    • Label clearly with withdrawal date and reason (optional)

  • 100% of enrolled children have complete, current files on record

  • Licensing inspections yield no file-related citations

  • Parents are notified proactively about missing or expiring documents
